How do U-50 syringe units work?

A U-50 syringe has 50 unit marks per mL, so 1 unit = 0.02 mL. The same 0.10 mL volume reads 10 units on U-100 but 5 units on U-50.

The math

mL = units ÷ 50
U-50 units = U-100 units ÷ 2

= 0.1 mL · concentration 2.5 mg/mL (2500 mcg/mL) · 20 draws per vial
Concentration = 5 mg ÷ 2 mL = 2.5 mg/mL
Volume = 250 mcg ÷ 2500 mcg/mL = 0.1 mL
Units = 0.1 mL × 100 (U-100) = 10 units
