What the columns are for
Date and time record when each entry happened. Amount is what your prescriber specified, written in the unit they used (mg or mcg). Units is the mark you drew to on the syringe. Site is a free-text box for whatever your clinician asked you to note, and Notes covers anything else, such as a new vial or a missed entry. The header has blanks for the month, the vial's concentration in mg/mL and the syringe size, so a reader can check that amount and units agree.
Everything is blank. The sheet doesn't suggest amounts, schedules or compounds; it's a place to record the plan you already have.
Checking units against amount (math only)
On a U-100 syringe, units = amount (mg) ÷ concentration (mg/mL) × 100. For example, at 2.5 mg/mL an entry of 0.25 mg corresponds to 0.1 mL, or 10 units. If a row's units don't match that arithmetic, recheck the concentration: it changes whenever you start a vial with a different water volume. The calculator and the units-to-mg page do the conversion both ways.
Printing and keeping it useful
- Print at Actual size / 100% on plain US Letter paper; any home printer works in black and white.
- Start a fresh sheet each month and write the concentration at the top; if you start a new vial mid-month, note it in the Notes column and update the header.
- Keep the sheet with your supplies or on a clipboard, and bring it to appointments so your prescriber can see the record.
- Use pen, not pencil, so entries don't fade or smudge.
What's in the Dose Log & Schedule
The set is four pages: the general units formula, this monthly log, a weekly grid with room for two entries a day, and a blank reconstitution worksheet (vial total, water, concentration, units, syringe size, mix and discard dates) with a capacity check. The Bundle adds the Vial Label Pack and the syringe units chart.
